By submitting this form, I acknowledge that I am voluntarily participating in the "Fit Defense" workout session(s) conducted by Ayers Academy and instructed by JR Ayers. These sessions will take place from 6:00 a.m. to 6:50 a.m. on Monday, April 28 and Tuesday, April 29, 2025, during the NCURA Region III regional conference.
I understand and agree to the following:
- Participation in fitness and self-defense training involves physical activity and carries the potential for injury.
- I am physically fit to participate and have not been advised otherwise by a qualified medical professional.
- I assume full responsibility for any risks, injuries, or damages, known or unknown, that I might incur as a result of participating in this activity.
- I knowingly, voluntarily, and expressly waive any claim I may have against Ayers Academy, its instructors, staff, affiliates, or the NCURA conference organizers for any injury or damages that I may sustain as a result of participation.
